I will recap your initial questions with the answers already given by my collegues and my additions to them. The answers to these questions might also be informational to the other players.
-Is the W/K/B ladder still in place?
-I saw someone banned yesterday without any kicks or warnings.. however giving the benefit of the doubt perhaps he was kicked before for it? That may have been however, the general server pop doesn't know that.

Adding to what Spare has said; warnings don't stop when the current round is over. If you've been warned last couple of rounds, you might not get another warning this round but you get kicked outright.
As Angerfist said; we are less lenient with repeated offenders. MoonGamers has a verbal policy Educate and not Eradicate. Which means is; we want to educate players on what the rules are and what not to do. However, some people cannot be educated or don't give a hoot, so they get kicked or as last resort banned.
Depending on the infraction, we might not give you another warning and we go straight to a kick. Let's say there is a chronic grenade spammer named ''X''. X has been warned and kicked for weeks already and he is still chronically spamming nades. At a certain point enough is enough and there are no more warnings. So an admin might kick X instantly.
Other players might think X got kicked without being warned first and think the admin is being harsh, but that is a lack of information on the players side which is totally normal. Thing like this is also a thing our players should not be worried about, that is why there are admins. Admins are here to make a problemless playing environment.
Aside from not having to worry about why a player got kicked, players may always ask an admin why a player got kicked even though the yellow text will show why a player was kicked.
I don't know what the player did that got banned that day, but depending on the infraction, benefit of doubt cannot always be applied. There are multiple rules which cannot be given benefit of doubt to (e.g. clan recruiting in the server).
-What about admins tagging up after issuing a command while smurfing?
IMO the question is kind of vague, but I hope this will give some insight and clear things up.
We let our admins give out general warnings/reminders without having to tag up (or while smurfing); one could think of a general ''don't taxi to the planes'' warning/reminder. When people are actually in need of some education or kick we like the admins to tag up. Usually the education has more value/meaning when the players know the person educating them under their normal (known) name.
The tagging up part is mostly bureaucratic for our Administration and partly showing the server population there is an active admin on duty. Admins will join the server tagged up when they are wanting to show the server pop there is an active admin around for questions and/or action needed. Admins might also come in with their normal name just to play, and can tag up if they are needed.
Luckily our players don't have to worry about if an admin is tagged up or not.

Legit questions, like I recapped and extended to above, should not get any grief in a thread like this (depending on how they are formulated or other topics that arise).
Observations like the one of the newer admins might be better (if at all) posted in a PM to one of the BF1942 senior admins, since it is a kind of a irrelevant thing to throw out into world and cannot be normally (if at all) responded to in a normal 'not getting grief over this' kind of thing.
In short; you might want to reconsider to what to post together in one single thread. As a part of this one is okay and an other part is an 'okay wtf, what is the relevance of this?' thing.
